Assist-Life Stage, Transition is usually funded under Capacity Building. Look for transition or life stage support in your plan, or ask your support coordinator.

How is this different from life skills development?

Transition support focuses on planning and coordination through a specific change. Life skills development focuses on building everyday skills over time. Many participants use both.

Can you work with my support coordinator?

Yes. We are happy to work alongside coordinators, families, and other providers involved in your transition.
